“Can you save it? I really don’t want to lose my tooth.”

Dr. Chelja Ram stared at the panoramic X-ray, her brow furrowed. The patient, a young man named Selvam, was sitting opposite her, his face pale and drawn. Selvam’s lower molar was severely damaged, the decay extensive.

“Dr. Chelja Ram,” Selvam pleaded, “Can you save it? I really don’t want to lose it.”

Dr. Chelja Ram understood the sentiment. Losing a tooth, especially at a young age, could have significant consequences – from cosmetic concerns to difficulty chewing and even shifting teeth. But the X-ray told a different story. The infection had reached the bone, and the prognosis for saving the tooth was bleak.

“Selvam,” Dr. Chelja Ram said gently, “The damage is extensive. While we can attempt a root canal, there’s a high risk of failure. The infection may persist, and we might end up having to extract it anyway.”

Selvam’s face fell. “But what are the alternatives? An implant? A bridge?”

Dr. Chelja Ram explained the options, outlining the pros and cons of each. Implants were the most natural-feeling but expensive and involved a longer recovery period. Bridges required grinding down adjacent teeth, potentially compromising their health.

“Ultimately,” Dr. Chelja Ram said, “the decision is yours. I can offer my expertise, but the final choice rests with you.”

 

The Dilemma of Preservation

Selvam sat in silence, weighing the options. He knew that preserving his natural tooth was ideal, but the risk of failure and the potential complications worried him.

Dr. Chelja Ram respected Selvam’s silence. She knew that this was a significant decision, and Selvam needed time to process the information.

“Take your time, Selvam,” Dr. Chelja Ram said. “Think it over carefully and discuss it with your family if you’d like. We can schedule a follow-up appointment to discuss your decision in more detail.”

As Selvam left the office, Dr. Chelja Ram couldn’t help but ponder the ethical dilemma. Was it always the right decision to pursue aggressive treatment to save a tooth, even when the odds were stacked against success?

She knew that every patient was unique, and every situation presented its own set of challenges.
